Public Notice: Severance payment applications & employers

The National Insurance Office is appealing to all those employers who received correspondence entitled Notice of Appearance by Respondent (Form 5) in respect of former employees, to complete and return these Form 5’s to the National Insurance Office.

This is urgently required to facilitate the continued processing of severance payment applications of former employees.

These forms should be placed in the drop-boxes at the Frank Walcott Building no later than Friday, March 19, 2021.

Please note that no scanned or email copies will be accepted.
